North Country Family Health Center Opens 5th Community Health Center Location in Lowville

Lowville, New York --- North Country Family Health Center (NoCo) has officially acquired Lowville Medical Associates, 5402 Dayan Street, Lowville, New York. It is now operating the practice as Lowville Family Health Center under its New York State Department of Health Operating Certificate. This is NoCo’s 5th community-based location; NoCo also operates 14 school-based locations, totaling 19 locations across Jefferson and Lewis Counties.
NoCo, a 501(c)3 organization, acquired Lowville Medical Associates, a private healthcare practice in December 2023 to ensure the long-term sustainment of primary healthcare services in Lewis County. In January 2024, all 23 staff members of the practice transitioned to North Country Family Health Center employees. During 2024, the practice underwent a variety of facility updates to align with NoCo’s Department of Health operating license. Last month, NoCo obtained its Operating Certificate from the New York State Department of Health for this location, making it NoCo’s second community-based location in Lewis County. NoCo’s other community-based location in Lowville provides dental and WIC services at 7785 North State Street in Lowville (within the Lewis County General Hospital).
“Patients will continue to see the same trusted providers and familiar faces they’ve come to rely on at the Dayan Street facility,” said Joey Marie Horton, Chief Executive Officer of NoCo. “Now, as part of the NoCo Community Health Center network, patients will benefit from access to a comprehensive, patient-centered model of care that includes a broader range of services and deeper support for their overall health and well-being.”
Also in late May 2025, NoCo transitioned the electronic medical record system at Lowville Family Health Center. The electronic health record, athenahealth, which is the same system utilized by all North Country Family Health Center primary care patients and allows patients to view their medical records, review test results, and visit summaries via a streamlined and easy-to-use patient portal.
“We welcome all the facility updates, the new electronic medical record system, and the name change that this year has brought to our group. We really want to stress to our patients that they will continue to receive the same high-quality care that they have relied on for decades,” comments Dr. Steven Lyndaker. “My colleagues and I are now able to provide services in a more modern, updated facility. But most importantly, our patients will now have access to a Community Health Center model of care, which will ensure high-quality healthcare for residents of Lewis County for decades to come.”
